Notice of Privacy Practices

This notice describes how health information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Who this notice is from

Basix Brands LLC ("Basix for Men") is a technology platform. We are not a healthcare provider and we are not a HIPAA covered entity. We connect you with licensed healthcare providers and with pharmacies, and we handle health information on their behalf and on yours.

The providers who review your intake and the pharmacies that dispense your medication are independent covered entities. Each maintains its own Notice of Privacy Practices, and that notice governs the information they hold. You can request theirs directly from them, or ask us at support@basixformen.com and we will tell you how to reach them.

We publish this notice so you can see how we handle your health information, and we apply the protections below whether or not the law requires us to.

What health information we handle

Your medical intake questionnaire and the answers you give
Medical history, current medications, and allergies you report
Photographs and messages you send to your provider
Your provider's treatment decision and prescription details
Refill status, shipment records, and check-in responses

How we use and disclose it

For treatment. We transmit your intake to a licensed provider so they can evaluate you, and we transmit an authorized prescription to a pharmacy so it can be filled. We pass messages between you and your provider.

For payment. We use order and subscription information to bill you. We do not send health information to payment processors.

For operations. We use health information to run the service: fulfilling orders, providing support, maintaining records, and improving safety and reliability.

As required by law. We disclose information when law requires it, including to public health authorities, in response to a valid court order or subpoena, and to prevent a serious and imminent threat to health or safety.

With your written authorization. Any other use or disclosure requires your authorization, which you can revoke in writing at any time.

What we never do

We never sell your health information
We never use your health information for advertising or marketing, and we never build advertising audiences from it
We never share your health information with an employer, insurer, or data broker
We never disclose your health information for marketing by anyone else

Your rights

Access. You can request a copy of the health information we hold about you.

Amendment. You can ask us to correct information you believe is inaccurate or incomplete.

Accounting of disclosures. You can request a list of the disclosures we have made, other than for treatment, payment, and operations.

Restriction. You can ask us to limit how we use or disclose your information. We will accommodate reasonable requests where we can.

Confidential communications. You can ask us to contact you a particular way or at a particular address.

Paper copy. You can request a paper copy of this notice at any time, even if you agreed to receive it electronically.

Withdraw consent. Where we rely on your consent to process consumer health data, you can withdraw it. Withdrawing consent may end our ability to provide treatment services to you.

To exercise any of these rights, email support@basixformen.com or write to us at the address below. We will verify your identity before acting. Requests about records held by your treating provider or pharmacy should go to them, and we will help you get there.

Breach notification

If your unsecured health information is breached, we will notify you as required by law.

Complaints

If you believe your privacy rights have been violated, email support@basixformen.com. You may also file a complaint with the US Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/complaints, or with your state attorney general. We will not retaliate against you for filing a complaint.
