Telehealth Informed Consent

Read this before you request treatment. You will be asked to agree to it as part of your medical intake.
What telehealth means here
Telehealth is the delivery of healthcare using electronic communication instead of an in-person visit. On this platform, you complete a written medical intake, and a licensed healthcare provider reviews it and decides whether treatment is clinically appropriate for you. Most reviews are asynchronous, meaning your provider evaluates your information without a live video or phone call. Your provider may request a live consultation, more information, or photographs before deciding.
Who is treating you
Your provider is a healthcare professional licensed in the state where you are located. They are independent of Basix Brands LLC, which is a technology platform and does not practice medicine. Your provider, not Basix for Men, is responsible for your care.
You have the right to know your provider's name and license information. Ask through the patient portal at any time.
What you are consenting to
By agreeing, you consent to:
- Evaluation and treatment by a licensed provider through telehealth
- Electronic transmission of your health information to that provider, to a pharmacy, and to the telehealth platform that connects them
- Asynchronous review, meaning your provider may evaluate you without speaking to you in real time
- Receiving a prescription, if authorized, and having it dispensed and shipped to you

Risks and limitations you should understand
No physical examination. Your provider will not examine you in person. They rely entirely on what you tell them. A condition that would be found during a physical exam or blood work may be missed.
Your answers determine the outcome. If the information you give is incomplete or inaccurate, your provider may reach a conclusion they would not otherwise reach. This can lead to a medication being prescribed that is not right for you.
Telehealth is not right for every condition. Your provider may determine that you need to be seen in person, and may decline to prescribe. Declining to treat is a legitimate clinical outcome, not a service failure.
Delays are possible. Technology can fail, and reviews can take longer than expected.
Privacy. We use encryption and access controls, but no electronic transmission is perfectly secure.
About the medications prescribed through this service
Finasteride is FDA-approved for male pattern hair loss. Reported side effects include decreased libido, erectile dysfunction, and decreased ejaculate volume. In some men, sexual side effects have been reported to persist after stopping. Finasteride can affect PSA test results, so tell any provider who orders a PSA test that you take it. Finasteride must not be handled by anyone who is pregnant or may become pregnant, because it can cause birth defects in a male fetus. There have also been reports of depression and mood changes.
Oral minoxidil is FDA-approved for high blood pressure. Its use for hair loss is off-label, meaning it is prescribed for a purpose the FDA has not approved. Off-label prescribing is legal and common, but you should understand that is what is happening. Reported side effects include unwanted hair growth elsewhere on the body, fluid retention, swelling, rapid heartbeat, dizziness, and lightheadedness. It is not appropriate for people with certain heart conditions.
Shedding. Increased shedding in the first few months of treatment is common and is not a sign that treatment is failing.
This is not a complete list of side effects or interactions. Read the information that comes with your medication and raise any question with your provider.
Emergencies
This service is not for emergencies and is not monitored around the clock. If you think you may be having a medical emergency, call 911 or go to the nearest emergency room. Do not use the patient portal to report an emergency.
Your rights
- You may withdraw this consent at any time, in writing, without affecting care you have already received
- You may decline telehealth and seek in-person care instead
- You may stop treatment at any time; tell your provider before you do
- You have the right to your medical records
- You have the right to know who is present in any live consultation
Withdrawing consent may end our ability to provide treatment through this platform.
Records and follow-up
Your intake, your provider's decision, and your messages are kept as part of your medical record and retained for the period required by the law of your state. Follow-up is available through the patient portal. Your provider may require a periodic check-in before authorizing further refills.
Cost
The price shown at checkout covers the medical evaluation, your medication if authorized, and shipping. We do not bill insurance. If treatment is not authorized, you receive a full refund.
